BE PREPARED TO PROVIDE SOME BASIC INFORMATION SO THAT YOUR CALL CAN BE TRAIGED. A DOCTOR or OTHER MEMBER OF THE TEAM - Pharmacist, Physio, Advanced Nurse Practitioner, Mental Health Practitioner, Social Worker all on site WILL RETURN YOUR CALL TO CARRY OUT AN ASSESSMENT.

PLEASE AVOID COMING TO THE SURGERY FOR ANYTHING OTHER THAN A PRE-BOOKED APPOINTMENT.

WHEN ORDERING YOUR PRESCRIPTION ON THE SCRIPT LINE PLEASE STATE YOUR CHOOSEN CHEMIST, DO NOT COLLECT AT THE SURGERY.
